A few points on starting a new story.

Generally speaking prophecy in the beginning of a story should always be fulfilled. But the best storys fulfill them in a way you would never expect.

Remember there are kinds of death not just the physical kind.

A psychological brake can be the death of one personality and the birth of another.
* Death of a good guy/ birth of a monster, is a common form.
* Less common and harder to write but more interesting death of a bad guy/ birth of a hero

There's death of innocence.
* Like the first time you see a crime.
* the first time a person is forced to kill.

Those can be great if well written.

It is the most important thing, to do for the reader what you say you are going to.
